Medical Travel Insurance - Masterassist Black

Mastercard Black Credit Cardholders can benefit from the security and protection offered by our comprehensive travel medical program. Some of the benefits provided are Medical Expenses, Emergency Medical Evacuation, Repatriation of Mortal Remains, Return of Dependents and Elders, VIP Transportation Services and Family Emergency Travel among other benefits.

Who Has Coverage:

Mastercard Black Credit Cardholders, their Spouses or Fathers and Dependent Children of the Cardholder, whether traveling together or separately.

To Get Coverage:

This coverage is automatically provided when the total cost of the Common Carrier's passenger fare has been charged to your Black Credit Mastercard and / or has been purchased with points earned by an eligible Rewards Program related to your Mastercard Black Card Credit (ie mileage points for travel). For an Insured Person to be eligible for coverage, you must pay all associated taxes, shipping and handling charges, and any other charges required with your Black Credit Mastercard and / or Rewards Points associated with your Mastercard Black Credit card.

Non-Cash Disbursement Services: All Emergency Medical Evacuations, Medical Expenses, Repatriation of Lethal Remains, Return to Residence location, Family Emergency Travel Costs, Return of Dependent Child / Elder, covered may be made "Without cash outlay" if Mastercard is notified at the point of service, which means that Mastercard can communicate directly with the Hospital or clinic and process the payment without the Cardholder making any disbursement.

Medical Expenses*:  If an Insured Person suffers an Injury or Sickness and needs medical care while away from their location of Residence, benefits are provided for Covered Medical Expenses (those listed in a-g and not excluded).

  • This coverage provides a maximum benefit of up to USD † 150,000 per Insured Person.

Covered Medical Expenses include:

  1. Charges made by a Hospital for room and board, floor nursing and other services, including charges for professional services, except personal services that are not medical, provided, however, that the expenses do not exceed the average charge of the Hospital For accommodation in semi-private room and food;
  2. Charges made by a Physician for diagnosis, treatment and surgery;
  3. Charges made for the cost and administration of anesthetics;
  4. Drug charges, X-ray services, laboratory analysis and services, use of radioactive and radioisotopes, oxygen, blood transfusions, steel lungs and medical treatment;
  5. Bandages, drugs, medications, and therapeutic services and supplies that can only be obtained with a written prescription from a physician or surgeon;
  6. Charges made for ground ambulance services;
  7. Dental treatment as a result of injuries to healthy and natural teeth, subject to a maximum of USD † 100 per tooth.

* Charges for the services listed above will not include amounts that exceed normal and usual charges for such services or those excluded.

What is NOT covered by "Medical Expenses" (Exclusions):

In addition to MasterAssist Black's General Exclusions, "Medical Expenses" benefits are not payable for any loss, fatal or otherwise, due to or as a result of:

  1. A Pre-existing Health Problem, as defined herein;
  2. Services, supplies or treatment, including any period of confinement in a hospital, which have not been recommended, approved and certified by a Physician as necessary and reasonable;
  3. Routine physical examinations (including child well-being care), diagnostic laboratory tests, x-rays or other medical examinations, when there are no objective indications or deterioration of normal health;
  4. Plastic or cosmetic surgery, except as a result of an accident;
  5. Dental treatment, except as a result of injury to healthy and natural teeth due to an accident;
  6. Glasses, contact lenses, hearing aids and examination for prescription or adjustment, unless an injury or illness has caused impairment of vision or hearing;
  7. Expenses incurred in connection with weak, tired or flat feet, corns, calluses or toenails;
  8. Diagnosis and treatment of acne;
  9. Expenses related to a deviated septum, including submucosal resection and / or other surgical correction of the septum;
  10. Organ transplants;
  11. Any mental, nervous or emotional disorder, or rest cures;
  12. Pregnancy: pregnancy-related illness or injury will not be covered if the illness or injury occurs within 3 months (three months before / three months later) at the probable (or actual) date of delivery.
  13. AIG (the Company) and / or the corresponding Reinsurer shall not be responsible for providing any coverage or making any continuation of payment if doing so implies being in violation of any law or regulation of sanctions that would expose AIG (The Company) and / or Reinsurer Its parent company or its subsequent controlling entity to any sanction under any law or regulation of sanctions.

Cash Benefit per Inpatient Patient: If an Insured Person is hospitalized as an inpatient, due to an Injury or Illness while away from their location of residence, a UDS benefit of † 200 will be provided for each day an Insured Person is Hospitalized up to a maximum 30-day confinement period, subject to a deductible of one (1) day. Hospitalization must have been recommended by a Physician.

"Hospitalized Patient" means an Insured Person who is confined to a Hospital on the recommendation of a Physician, and by whom the charges for room and hospitalization are made.

Emergency Medical Evacuation / Repatriation: This coverage will provide benefits for covered expenses incurred 100 miles (160 kilometers) or more away from the Insured Person's residence or outside the Residence location of the Insured Person, in the event of That an Injury or Illness that begins during the course of a Covered Trip causes the need for an Emergency Medical Evacuation of the Insured Person.

An Emergency Medical Evacuation means that (a) a legally authorized Physician certifies that the severity of the Insured Person's state of health justifies the immediate transportation of the Insured Person from the place where he / she is injured or sick to the nearest hospital so that he / Receiving appropriate medical treatment; Or (b) after having received care in a local hospital, the Insured Person's state of health justifies transportation to the current place of residence for medical treatment or recovery; Or (c) in the case of both situations described in (a) and (b) above. We will make the necessary coordination for any medical assistance and Emergency Evacuation services. Decisions regarding the level of urgency of the case, the timing and the most convenient means of transportation will be made in consultation with the local Physician who is attending the case and the Physician who will receive the patient.

Limitations / Conditions of Coverage:

  • The expenses for special transportation must be: (a) recommended by the Physician who attends the case or (b) required by the standard means of transport regulations to transport the Insured Person. Special transportation includes, but is not limited to, air ambulances, ground ambulances and private motor vehicles.
  • Covered expenses include expenses incurred for transportation, medical services, and medical supplies incurred in connection with the Emergency Evacuation of the Insured Person.
  • Expenses for medical supplies and services must be recommended by the attending Physician or required by the standard means of transportation regulations to transport the Insured Person.
  • All transport arrangements made to evacuate the Insured Person must be carried out in the most direct and economic way.

Returning to the location of Residence before a need: If an Insured Person is hospitalized outside their location of Residence, for more than 24 hours, benefits can be provided to pay the cost of an air ticket to return to the Cardholder, Spouse or Partner of Fact or accompanying person to the location of residence or destination that it is considered necessary to accompany the Cardholder during an Emergency Medical Evacuation.

Hotel Convalescence Costs: If an Insured Person is Hospitalized for more than three (3) days and then must stay, on a medical recommendation, in a hotel (outside of their location of Residence before discharge), while outside their location of Residence, benefits are provided beginning immediately after you have left the Hospital for additional convalescence.

  • This coverage provides benefits of up to USD † 250 per day, up to a maximum of five (5) days.

Family Emergency Travel Costs: If an Insured Person travels alone and is hospitalized for more than three (3) days while away from his / her location of Residence, benefits may be provided for one (1) immediate family member or friend Chosen by the Insured Person travel to and from to remain next to the Insured Person. Benefits are available to pay for the cost of airfare and daily hotel accommodation charges for a maximum of five (5) days.

Return of Dependent Child / Elder If an Insured Person is hospitalized for more than three (3) days, during a Covered Trip and is 100 miles (160 km) or more from his or her residence, or when outside of his or her location of Residence, Benefits may be provided for (a) the return home of the Child (s) or Elder (s) accompanying him on the Trip and remaining alone; And / or b) the coverage of expenses for a person to go and return accompanying the elderly or dependent minors on their return trip home.

  • This coverage provides a maximum benefit of up to USD † 25,000 for expenses incurred.
  • The benefits will not be provided for any expenses provided at no cost by a third party to the Insured Person or already included in the cost of the trip.
  • The maximum benefit of up to USD † 25,000 is the maximum amount to be paid regardless of the number of Dependent Children or Elderly eligible on the Covered Trip.

VIP Transportation Expenses If an Insured Person is hospitalized outside of their location of Residence, benefits are provided to reimburse the costs of limousine, taxi or other automobile services provided to the Insured Person, Spouse or Dependent Partner and / or Dependent Children for:

A) To move between the hospital and the hotel; I

B) Transfer between the hotel and the airport in order for the Insured Person to return to his / her location of Residence upon discharge from the Hospital.

Home Assistance in Your location of Residence: In the event that the Insured Person suffers an Injury or Illness during a Covered Trip outside of their location of Residence, the Company will pay up to USD 150 for home help in their location of Residence, provided what;

Exclusions (Home Assistance in your location of Residence):

In addition to the General Exclusions listed in the Benefits Guide, this coverage will not include:

  1. Any claim arising out of a travel taken within the location of Residence of the Insured Person.
  2. AIG (the Company) and / or the corresponding Reinsurer shall not be responsible for providing any coverage or making any continuation of payment if doing so implies being in violation of any law or regulation of sanctions that would expose AIG (The Company) and / or Reinsurer Its parent company or its subsequent controlling entity to any sanction under any law or regulation of sanctions.

Pet / Kennel Boarding Charges: If the return of the Insured Person to his / her location of Residence is delayed after an Injury or Illness while outside his / her location of Residence, benefits may be provided for extra boarding and residence charges Pets, cats and dogs, provided that the Insured Person has a valid Medical Expense covered under MasterAssist Black.

  • This coverage provides a maximum benefit of up to USD † 500 per incident.

Apart from the General Exclusions, coverage is not available:

  • If the cat or dog of the Insured Person was not in their shelter or day care for the duration of the Covered Trip.
  • Any claim arising out of a travel taken within the location of Residence of the Insured Person.

Repatriation of Mortal Remains: If the Insured Person suffers a loss of life being 100 miles or more (160 kilometers) away from his / her place of residence or outside his / her location of Residence, benefits are provided for reasonable expenses for the conservation and Return of the body to house.

  • This coverage provides a maximum benefit of up to USD † 100,000 per insured person.

What is NOT covered by MasterAssist Black (General Exclusions):

This insurance plan does not cover any loss, fatal or not, caused by or as a result of:

  1. Suicide or any attempt of suicide by the Insured Person, being in their right mind, or self-destruction or attempt of self-destruction by the Insured Person, being insane;
  2. A loss caused directly or indirectly, in whole or in part, by medical or surgical treatment, except as necessary solely as a result of an Injury;
  3. Participation in the military, naval or air force service of any location;
  4. The use, release or escape of nuclear material that results in, directly or indirectly, a nuclear reaction or radiation or a radioactive contamination; Or the dispersion, release or application of pathogenic or poisonous biological or chemical materials;
  5. Being under the influence of or having any connection with drugs or toxic substances unless they are swallowed on the recommendation of and as prescribed by a Physician; And / or drug abuse, addiction, or overdose;
  6. Participation in a real crime;
  7. Participation in any sport of professional, semi-professional or interscholastic equipment;
  8. Participation in speed contests using a motor vehicle;
  9. Participation in acrobatic parachuting / parachuting, hang gliding, bungee jumping, mountaineering (this does not include normal recreational mountaineering or similar activity), caving;
  10. A trip against the advice of a Physician.
  11. Definitions - Master Assist Black

Accident: refers to a sudden, unpredictable, uncontrollable and unexpected physical event that occurs to the Insured Person caused by external, violent and visible means during the Covered Journey.

AIG (The Company): means the Company that signs the Insurance Coverage.

Up-to-date: means an Account that is up-to-date on your fees and payments and that follows all other requirements of the issuer.

Elderly: means an adult, male or female, over 65 years of age

Travel Companion: Means the person traveling with the eligible Cardmember in the same time of travel.

Preexistent Condition: for an Injury, it means a health problem for which medical care, treatment or advice by a Physician was recommended or received, or was first manifested or contracted prior to the commencement of a Covered Trip. For a Sickness, it means a health problem that occurs during the period of ninety (90) days prior to the commencement of a Covered Trip, for which treatment has been sought or recommended by a licensed physician, or for which symptoms Which would make a prudent person seek a diagnosis, care or treatment. A health problem will not be considered a Pre-Existing Condition if it is controlled (it does not show symptoms or require adjustment of treatment or medications) throughout the ninety (90) day period due to ingestion of drugs or prescription drugs, and For which travel restrictions are not recommended by a Collegiate Physician.

Issuer: means a Bank or financial institution (or similar entity) or Corporation and / or Government Institution that is admitted and / or authorized by Mastercard to operate a Mastercard program in the Mastercard Black Credit issuance Territory.

Illness: means illness or illness of any type contracted and / or that has begun during a Covered Trip.

Dependent Child (s): means named dependent children, including adopted children, and stepchildren, of the cardholder, ranging from newborns to 18 years of age, or 25 years of full-time attendance to an accredited Higher education, who are single and permanently resident with the Cardmember, and receive a majority of their support and support from the Insured Person.

Children who are fully dependent on the Insured Person (s) for support and maintenance due to a permanent physical or mental disability shall be deemed dependent upon the Insured Person (s) If the Insured Person (s) is entitled to benefits.

Hospital: means a place that (a) has a valid license (if required by law); (B) operates primarily for the care and treatment of sick or injured persons; (C) has a staff of one or more Doctors available at any time; (D) provides 24-hour care and has at least one registered nurse on duty at all times; (E) has installed surgical and diagnostic facilities, either on the premises or in facilities available to the Hospital or on a pre-agreed basis; And (f) is not, incidentally, a clinic, nursing home, nursing home, or nursing home for the elderly or a facility that functions as a drug and / or alcohol treatment facility.

Common means of transport: means any carrier by land, water or air operated under a valid and up-to-date license for the carriage of passengers by contract for which a ticket has been obtained.

location of Residence: means the location where an Insured Person has a fixed, real and permanent home, as well as its principal place of business, and to which it intends to return, which must be the same as the Mastercard Black Credit Card Issue Territory. For purposes of MasterAssist Black, travel from a territory or province or island of the United States to another territory or province or island of the United States is considered a trip outside the location of Residence.

"Facto couple" means a couple of the same sex or opposite sex, who has fulfilled all the following requirements for at least 12 months: (1) remains together with the Insured Person in the Cardholder's principal residence; (2) shares the financial assets and obligations with the Cardholder; (3) is not related by blood ties with the Cardmember to a degree of closeness that prohibits a legal marriage; (4) is at least of age in the state in which they reside; And (5) neither the Cardholder or Facto Couple is married to any other person, nor has any Facto Couple.

Insured Person (s): Refers to a Mastercard Black Credit Cardholder as well as any other person identified as eligible in the "Who Has Coverage" section of each program in this guide.

Policy: means the insurance contract and the attached endorsements or attachments issued to Mastercard

Points or Reward Program: means a program developed / offered by Mastercard and the issuer, which allows Cardholders to accumulate value (miles points, cash, etc.) and redeem rewards (merchandise, travel, etc.) on a Mastercard eligible. Eligible Rewards Programs must be the result of transactions made with a "plastic" card associated with Mastercard, regardless of the value earned (points per mileage, cash, etc.). Reward Programs in which points are not generated by transactions made with a "plastic" card, or are not associated with Mastercard at all, are not considered eligible Rewards Programs. Cardholder: means a person who has an eligible Account and whose name is on the surface (recorded or otherwise) of a Mastercard Black Credit card, provided by an Issuer in the Issuing Territory.

Covered Trip: means a trip where (a) the insured person's full rate has been charged to an eligible Account or (b) purchased with an eligible card that received points for miles, and other similar discounts for travel Rewards issued by Mastercard or an International Mastercard Issuer, or (c) both points (a) and (b) of this definition and by an Insured Person on behalf of another Insured Person. How to File a Claim

MASTERASSIST BLACK *

Notification Period of the Claim: Within ninety (90) days after the date of the loss.

Presentation Period: Not later than one hundred eighty (180) days after the date of Notification of the Claim.

Required Information (proof of loss):

1. Documentation detailing the nature of the injury or illness with a breakdown of expenses, including certified copies of: medical reports, statements by the attending physician, medical receipts, and related documentation;

2. Verification of the transaction confirming that the passenger fare for the Covered Trip was fully charged to your Black Credit Mastercard card, including copies of the Common Carrier's ticket (s) and receipts;

3. The cardholder's statement showing that you are eligible according to the definition of "Account" in the Key Terms and Definitions section (General)

* MasterAssist Black offers assistance for services without cash disbursement. Payment of Claims:

Where permitted by law, the Loss of Life Benefit is payable to the beneficiary designated by the Insured Person. If there is no such designated person, the payment of the claim shall be made to the first beneficiary who survives the Insured Person, as follows:

A. Spouse or Partner of Fact;

B. Children, in equal parts;

C. Parents, in equal parts;

D. Brothers and sisters, in equal parts; or

E. Excuse the administrator.

Subject to the applicable terms and conditions, all other benefits will be paid to the Insured Person or to another corresponding party when necessary. The payment of any indemnity will be subject to the laws and governmental regulations that are in force in the location where the payment is made.

† Each insurance benefit limit described in this Guide is expressed in United States Dollars (USD). The payment of claims will be made in local currency when required by law, with the official Foreign Currency Exchange Rates published on the date on which the payment of the claim is made.

General Provisions and Legal Notices

General: These benefits and services are effective as of January 1, 2016 for eligible Mastercard Cardholders. This document replaces any program guide or communication you may have previously received. The information contained in this document is provided for the sole purpose of providing general information. This document is not a Policy, nor a contract, nor a guarantee, nor a promise of insurance or other benefit. It is not intended to be a complete description of all terms, conditions and exclusions of the policies or other benefits, all of which are subject to change by Mastercard or the insurers or other service providers, at any time and without notice . The provision of services is subject to availability and legal restrictions. Travel Assistance Service is provided through AXA Assistance, USA. UU. This insurance coverage is insured by AIG Insurance Company Approved Member Companies. The complete provisions related to these insurance plans are included in the Master Policy (s) found in the files of the Plan Administrator, Affinity Insurance Services, Inc. on behalf of Mastercard Worldwide, Latin America and the Caribbean Region, in Purchase, New York, USA. UU. If there is any discrepancy between this document and the Master Policy (s) or the corresponding Mastercard agreement for other benefits, the Master Policy (s) or the applicable agreement Of Mastercard for other benefits. The Insurance Company is the one that has the final power to decide the result of an insurance claim. The designated service provider for non-insurance services and benefits provided to cardholders has the final authority to determine and respond to any claim, comment, query or dispute relating to the use of the Cardholder benefit program.

Cancellation: Mastercard may cancel these benefits at any time or choose not to renew the insurance coverage of all Cardholders. The coverage or insurance benefits provided by these programs may terminate on the date indicated in the Master Policy between Mastercard International and the Insurance Company; Or terminated on the date your Mastercard is terminated or ceases to be a Valid Account, whichever occurs first. If a benefit / program is canceled, cardholders will be notified in advance, as soon as reasonably practicable. The effective time for any Cancellation referred to in this section will be 12:01 a.m., Eastern Time. Any claim for benefits of MasterAssist Black occurring before the effective date of cancellation will not be adversely affected by cancellation or non-renewal, subject to the terms and conditions of the Master Policy (s) S).

Valid Account: (1) In order to enjoy benefits, coverage or services, your Mastercard card account must be open, valid and current; And 2) If, on the date of an accident, event or incident that causes or results in a loss covered by an insurance plan, your account is not open, valid and up to date, or incurred in arrears, collection, or Cancellation, the Benefits will not be paid and the coverage will not apply.

Cardholder's Duties After a Loss (Due Diligence): You must use all reasonable means to avoid future loss at the time and after a loss occurs, and to fully comply with all of the duties described. This includes full cooperation with the Insurance Company, the Plan Administrator, and the Third Party Claims Administrator acting on behalf of Mastercard for programs or other services offered as benefits to a cardholder in the investigation, evaluation and payment of a claim.

Concealment, Fraud or False Disclaimer: The coverage / benefits or insurance services described in this document will be voided, either before or after a loss or to make a service request, if you have intentionally concealed or distorted any Fact or circumstance, or has provided fraudulent information about the insurance plans or other services described herein, to: Mastercard International, the Insurance Company, the financial institution issuing the Card Account or any other Company providing services and / or administration on behalf of these programs.

Legal Actions: No legal action can be filed to recover on a Policy until sixty (60) days elapse after the Insurance Company has received Written proof of loss. Said action may not be filed after three (3) years from the time it is required that the proof of loss be submitted in writing.

Compliance with local statutes: Any provision of a Policy that, as of its effective date, conflicts with the statutes of the location in which the policy was delivered or issued for delivery is hereby amended to comply with Minimum requirement of those statutes.

Sanctions: AIG (the Company) and / or the corresponding Reinsurer shall not be liable to provide any coverage or make any continuation of payment if doing so implies being in violation of any law or sanction regulation that would expose AIG (The Company) and / Or reinsurer, its parent company or its subsequent controlling entity to any sanction under any law or sanction regulation.

Arbitration: Any dispute related to the terms of any Insurance Policy, including any questions related to its existence, validity or Termination shall be subject to and resolved by arbitration, and in accordance with the arbitration rules / regulations of the location where your Mastercard card account has been issued.

Confidentiality and Security: We may disclose all information we collect, as described above, to companies providing administrative services on our behalf only in connection with the insurance coverage you have received. We limit access to personal information to our employees, employees of our affiliates, or to others who need to understand that information to service the account or during the normal course of our business operations. To protect your personal information, we maintain physical, electronic, and procedural safeguards.
